Who needs power of attorney?
- Persons who don’t have parental responsibility or guardianship for a patient and are submitting applications or acting on behalf of the patient
- Institutions that apply for reimbursement of travel expenses in connection with a patient’s travel for example, an asylum reception centre or a school that has paid for travel expenses for a patient and is applying for coverage of these expenses
A person with parental responsibility or guardianship does not need power of attorney to seek or act on behalf of the patient for whom they are responsible.
Digitally-issued power of attorney
You can give power of attorney by logging in and selecting proxies under "Profile and settings". Here you can either add a family member from the list, or register another person who you want to give power of attorney. You can decide whether a digitally-issued power of attorney is valid for a limited or unlimited period.
Power of attorney by post
If you send a letter granting power of attorney by post, the letter must clearly indicate this fact, and contain the following:
the name, contact information and social security number of the person giving power of attorney and of the person who is being granted power of attorney. You must also state the scope of the power of attorney app
The letter granting power of attorney must be attached to the application and will be kept on your file for each case. The Patient Travel Office does not have the capacity to store these letters, and a new letter must be attached to every application submitted to the Patient Travel Office.
Use the power of attorney letter template when making a physical application by post.
